The Clinical Study of Jaundiced Patient Due to Distal Common Bile Duct Obstruction

Abstract
We operated 32 cases of jaundiced patient due to distal common bile duct obstruction for 4 years, of which 8 cases were benign stricture of the ampulla of Vater, 9 cases sandy and muddy stone, 3 cases recurrent stone and 12 cases periampullary neoplasm respectively.
The results were as follows:
1. On physical examination and symptoms, 18.7% of all patients had nausea and vomiting, 75% of all patients had abdominal pain in the right upper quadrant and in 58% of periampullary neoplasm patients the right upper quadrant mass was palpated.
2. The greast incidence of the disease occurred in the age group of fifth decade which comprised 28.1%.
3. Cholecystoenterostomy is a simple and effective means for bypassing the bile duct and deserves greater use in the distal common bile duct obstruction; as a second operation for choledocholithiasis or benign stricture of ampulla of Vater. Cholangitis is very rarely a sequel to this anastomosis.
4. The survival following nonfatal operations of periampullary neoplasm was relatively longer with cholecystojejunostomy (mean 9 months) compared with external drainage (mean 5.2 months) and exploratory laparotomy (mean 2.5 months) respectively.
